Dr. Leslie Yedor works with your current healthcare team and is not a replacement for your primary care physician. All new patients are required to have a current primary care physician to cover emergencies, routine care, and screening.
Functional Medicine services, acupuncture, and other therapies used as part of your treatment plan may or may not be covered by your insurance plan. Please check with your insurance company prior to your visit. Our office does not bill your insurance directly but we will provide you with an itemized receipt for services which you may submit for reimbursement. We are not a participating provider of Medicaid or Medicare.
We operate as a self-pay clinic and payment is expected at the time of service. If you have any questions please feel free to contact us.
Cancellation Policy
Please provide at least 48 hours notice for an appointment cancellation. Cancellations with less than 48-hours notice or no show visits may be charged $50 for a missed appointment. If you have questions or concerns regarding our cancellation policy please feel free to contact us. In order to keep our clinic running on time, please arrive 10 minutes before your scheduled appointment time.
2019 Fee Schedule
• Functional Medicine appointments – $1300 for two visits.
• After a year as our patient, the rate changes to $1300 for four visits.
• Initial 90-minute Physical Medicine Visit $225
• Follow-up 60-minute Physical Medicine Visit $175
Please inquire for information about long-term care plans, in-home care, and private functional lifestyle retreats.

of your initial and/or follow-up visit(s).
» Longer or shorter appointment times may be recommended by the doctor depending on your treatment plan and individual needs. Such appointments are billed at $45 per 15-minute increment.
» Student, Senior, EMS provider, and other discounts may be available upon inquiry.
*Acupuncture Visits are available to established Functional Medicine patients as an adjunct therapy.
